Our Practice administrator will work with you to ensure that your insurance claim is filed properly and accurately. Most insurance companies cover physical therapy costs; although specific coverage will vary depending upon your particular policy. If you have questions about your policy it is always best to contact your carrier for the most accurate and up to date information.
Proformance Physical Therapy participates with most major insurance companies and networks including BlueCross BlueShield, United Healthcare, Coventry, Cigna, Midlands Choice, Medicare, Medicaid and many more. If you’re covered by another carrier and are unsure about whether we’re a participating provider, please feel free to call our Practice Administrator.
When you arrive for your first visit, we will verify that your plan covers physical therapy as well as gather information about your specific benefits. Most insurance plans require that you have a doctor's referral for physical therapy and some insurance plans require pre-authorization before treatment begins. If you have any questions about your insurance plan contact your carrier or give us a call at 402-327-9000 and we will be happy to assist you in any what that we can.
Please bring your insurance card and the prescription written by your doctor (if you have one). If you do not have insurance, a prescription from a doctor is helpful but not required.
Arrive 10-15 minutes early to fill out this paperwork in our office. Please complete all paperwork before or at your first visit.
Dress appropriately. If you are being seen for a knee injury please wear or bring shorts; if you are being seen for a neck or shoulder injury, a tank top is helpful; if your treatment consists of exercise, please wear close-toed tennis shoes and appropriate exercise clothing. Additionally, if you have been wearing a brace, orthotic or using an assistive device, please bring those as well. On your first visit your physical therapist will perform a comprehensive physical therapy evaluation and develop a personalized treatment plan to optimize your return to health and help you achieve your goals. We will discuss your condition and the expected healing process with you. We believe that knowledge is power. Knowledge and understanding of your symptoms and your physical health will speed recovery and prevent future injuries.
Your first visit will include an initial evaluation and treatment session which will last approximately 1 to 1½ hours. Subsequent visits will average one hour depending on the location and nature of your injury. Following your initial evaluation, your physical therapist will be able to give you a more accurate time frame for future treatments.
Absolutely. We want to accommodate your schedule; therefore, we have morning and evening hours available. Proformance Physical Therapy is open MONDAY THROUGH FRIDAY 7:00AM TO 7:00PM
Our physical therapists are in constant communication with referring physicians. After your first physical therapy appointment an evaluation will be faxed to your medical doctor. We will re-evaluate your condition on an ongoing basis and send progress reports to your doctor. We ensure that a progress report will be at your doctor’s office for your follow-up visit. We will keep your physician up to date on your physical therapy progress and status.
Please feel free to ask your doctor if physical therapy is right for you. If your doctor is not sure, he or she is welcome to contact us regarding the benefit you will receive from physical therapy. Please be aware that you have the right to choose where you would like to go for your physical therapy. If you have a problem that you think can be helped with physical therapy, you can also give us a call and we would be happy to discuss your specific situation with you and let you know if it can be helped through physical therapy.
Your success in physical therapy depends on you. We will provide you with the necessary means to your recovery, but you are ultimately responsible for your health. In order to expedite your recovery, we ask that you be as consistent with your attendance in physical therapy, give your best effort, and follow through with your physical therapists’ recommendations for your home management program. Communication between you and your physical therapist is important. We welcome any questions you have about your program.
