We will send out paperwork a few days before your appointment. These documents can be completed and returned electronically with no need to print. However, should you have issues with this process or want to fill out paperwork before you receive an email from us, our forms are listed below. You may save the completed files to your computer and email them to info@proformancetherapy.com or print and bring them with you to your appointment.

Depending on your insurance and service you are seeking, additional forms may be required upon your arrival. Please also bring your physician's referral/script (if applicable) and your insurance card(s).

*If your appointment involves custom orthotics or a worker's comp/personal injury claim, we ask that you please fill out the respected paperwork in addition to your new patient forms.
